the SERVICE ADDRESS (this is usually found on the first page) and also
the SIGNATURE page from your lease agreement. You must upload these
pages as part of this online application submission.

How It Works
When you contribute to the UtiliCare program, your donations go to help families
in Jackson, TN when they need utility assistance. Unique situations arise that
make maintaining utility services more difficult for families from time to time.
UtiliCare makes it easier for customers like you that would like to help those less
fortunate and facing economic hardships in our community.
The contribution you select for UtiliCare, either a one time donation or a monthly
recurring donation, will be added to your Jackson Energy Authority utility bill. This
donation will be disbursed to Southwest Human Resource Agency (SWHRA) to
be used exclusively for JEA customers needing utility assistance.

profit agency that serves an eight county region including Madison County.
SWHRA takes a holistic approach to meeting the needs of individuals by
providing utility assistance and training opportunities.
You authorize us to use the phone number provided to verify your identity, which may include disclosing the phone number provided to a third-party to send you a One-Time Password via SMS text message. If a One-Time Password is sent, mobile messaging rates may apply. You authorize your wireless operator to disclose to us your account, subscriber, device and billing information if available, to support verification of your identity. Where applicable, this information may also be shared by us with other companies to support your transactions and for fraud avoidance purposes.
The applicant making application for utility services at the requested
service address agrees to pay for said services as measured by the Jackson Energy Authority (JEA) meters
according to rates applicable.
The applicant agrees to permit authorized agents of the JEA free access to the premises of the consumer
for the purpose of inspecting, reading, repairing or removing property of JEA. The applicant agrees that
any cost involved in collection of past due balances on this account including, but not limited to,
collection agency fees, attorney fees and court costs will be paid by the applicant.
JEA shall have the right, but shall not be obligated, to inspect any installation before Electric, Gas, Water
and/or Wastewater service is introduced, or at any time, and reserves the right to reject any wiring or
piping. JEA will not be liable or responsible for any loss or damage resulting from defects in the
installation, wiring, piping or appliances, or from violation of JEA Rules and Regulations, now in force or
those which may hereafter be adopted, or from accidents which may occur upon customer’s premises.
The applicant agrees that this application is subject to the JEA Rules and Regulations, now in force or as
may be adopted, copies of which are open for inspection at the business office of JEA; and that such
Rules and Regulations are a part of this Agreement and incorporated herein by reference.

Federal law protects your privacy rights as a customer of JEA. These
rights are in addition to the existing safeguards that JEA already has in place to ensure one’s privacy
rights. The Federal Communications Commission (FCC) requires JEA to notify all subscribers of their
rights to restrict the use of their Customer Proprietary Network Information (CPNI). You have the right,
and JEA has a duty, under federal law, to protect the confidentiality of your CPNI.
What is CPNI? Customer Proprietary Network Information (CPNI) is personally identifiable information
that a telecommunications carrier collects when providing telephone service to a subscriber. CPNI
typically consists of telephone numbers called by the subscriber along with the associated call duration
and timing of those calls as well as call frequency and any call management services utilized by the
customer.
Why should I care about the use of my CPNI? Congress has recently enacted federal legislation making a
practice known as “pretexting” a crime. This practice involves a party, acting under the false “pretext” of
being a customer, contacting a telephone company or accessing on-line accounts and obtaining CPNI,
often times with the intent to sell it to others. Federal CPNI protection rules have existed since 1996 but
because of the development of “pretexting” and other unethical practices for misuse of CPNI new rules
are now in place for your protection.
Permitted Use of CPNI By JEA Without Your Permission - CPNI can be used by JEA for certain purposes
without your permission. JEA may use CPNI to offer you new or enhanced services that are related to
the category of services to which you currently subscribe. JEA may also use CPNI to respond to your
inquiry regarding services you currently use or related services JEA offers. In addition, JEA may use CPNI
in connection with repair and maintenance services, billing and collection, and to protect company
property and to prevent fraud.
Prohibited Use of CPNI Unless Authorized By You - Unless you specifically authorize its use, JEA may not
use CPNI to market services unrelated to the services to which you currently subscribe. For example, JEA
may not use CPNI to offer you any type of long distance service unless you currently subscribe to their
long distance offerings. JEA may not share CPNI with any other company, including our affiliate
companies unless you are a customer of our affiliate.
Additional Information Regarding Your CPNI Rights - You have the right to deny or withdraw access to
CPNI at any time or to instruct JEA to disclose CPNI to unaffiliated third parties upon submission of a
written request. Any approval or denial for the use of CPNI outside of the service to which you
subscribe to from the company is valid until you affirmatively revoke or limit such approval or denial. A
denial of your approval will not affect the provision of any services to which you subscribe.
The applicant making application for telephone service, gives permission for the use of CPNI by JEA for
those uses specifically permitted with my consent. I understand I may limit or revoke this authorization
at any time upon proper written notice to JEA.
I understand that an installation fee of $100.00 will be waived only if the service is retained active in my
name for a minimum period of twelve (12) months continuous service. Early termination prior to the
twelve (12) month period will result in the fees being applied to my final telecommunications bill.
I understand that a battery backup will be
installed by JEA in the equipment box which terminates JEA’s fiber network, generally located on the
outside of the home or business. This Battery Backup enables your JEA telephone service for E911 calls
to function during a power outage for approximately 8 hours. The backup battery should give you 6
hours of talk time. In the event of an outage, limit unnecessary phone calls to preserve battery life. As
part of your service, we maintain your backup battery on your behalf, and will replace any
nonfunctioning battery at no cost to you. A 24-hour battery backup is available for an additional cost. If
you would like the 24-hour battery backup at an additional cost, please add this as a note in the
Comment section of the web form application.
Applicant, making application for propane services, do affirm that I either own or rent the property
located at the requested Service Address. I agree to become a ProGas customer and buy my propane
through Jackson Energy Authority (JEA).
I further agree to allow JEA access to any facilities at this location that are owned, furnished or operated
by JEA, including tanks, lines, meters, regulators or any equipment that would be necessary to provide
propane for my use. I further authorize JEA to remove the equipment should I discontinue the use of its
products or services with a 30-day written notice at any time after the initial 12-month service period. I
understand that it is my responsibility to contact my current provider for the removal of any equipment
that it may own. This Agreement continues in effect for a period of one (1) year from the date of initial
availability and thereafter is month to month unless thirty (30) days written notice is given by Customer
or JEA to terminate the Agreement.
As a ProGas customer, you can choose to be a metered customer or a bulk customer. Please add this as
a note in the Comment section of the web form application.
Metered
As a metered customer, I understand I will pay for my propane as I use it on my monthly utility
bill. I also agree to pay a “Customer Charge” at the rate determined by JEA. I understand that this charge
also will appear on my monthly bill during that time period. I also understand that if I use less than 100
gallons of propane per year for two consecutive years I will be classified as a Standby Customer and will
be charged a tank rental fee for the year.
Bulk
As a bulk customer, I understand that I will pay for my propane when JEA refills my tank. I also
agree to pay an annual tank rental fee as determined by JEA. I also understand that as a bulk customer I
will not be entitled to traditional service privileges. As a bulk customer, I can choose to have JEA refill my
tank automatically or to call JEA when I need a refill.
JEA's Facilities
All tanks, pumps, piping, hoses, fittings, adaptors, signage and other facilities necessary
to provide the propane supplied, shall be furnished and installed on the Customers premises by JEA and
shall remain the property of JEA. Customer shall provide suitable space for JEA’s facilities and shall
protect them from damage, injury, and or theft, and indemnify JEA for any and all damage, injury and/or
theft, and shall permit no person other than an agent of JEA, or a person otherwise authorized, to
tamper with, inspect or remove the same. Customer agrees that JEA representatives shall have the right
of free use, ingress and egress to such equipment at all times for the purpose of installation, operation,
repair or removal. I agree to abide by the policies and procedures of JEA.
Applicant, making application for natural gas services, do affirm that I either own or rent the
property located at the requested Service Address. I confirm that all connected natural gas
equipment on the premises is designed for, and will meet, all Federal, State and local codes.
I agree to provide a listing, or mechanical drawings that include BTU ratings of all connected
natural gas equipment installed at the premises. I understand that it is my responsibility to
notify Jackson Energy Authority if any changes occur in the total connected load.
I understand that Jackson Energy Authority provides a standard delivery pressure of seven
inches of water column (approximately 0.25 psi) for natural gas. I also understand that the
feasibility of providing a delivery pressure other than standard pressure and the type of meter
and pressure regulation required shall be determined by JEA.
Upon request for non-standard delivery pressure, I agree to confirm the verification of any and
all natural gas fired equipment that requires higher than standard pressures. I understand that
pressure above 5 PSI and piping sizes 4” and above will require welded pipe. I further agree to
provide all necessary downstream pressure regulation to protect facilities.
Following the request for non-standard delivery pressure, I understand that reasons for denial
of the request could include, but are not limited to the following:
Failure to meet the criteria outlined in the agreement
Jackson Energy Authority’s safety concerns regarding the non-standard natural gas pressure request
Lack of capacity of Jackson Energy Authority’s distribution system to deliver the requested pressure
I further understand that approval of non-standard delivery pressure does not guarantee the
pressure will be available under all conditions. I have been made aware that certain system
conditions could make the desired pressure unavailable, such as extreme cold weather,
emergency situations, maintenance, etc.
