- Customer Information:
- Account Name – Name of account holder
- Service Address – Location where services are used
- Account Number – 12-digit reference number unique to every Jackson Energy Authority customer
- Cable Services:
- The billing period for cable services
- The itemized list of cable services including any package discounts that may apply
- Optional Cable Services:
- The itemized list of optional services added to your cable service
- Cable Taxes/Fees:
- Sales Tax – Tax for the amount billed
- FCC FEE and Franchise Fees – Amount billed for government fees
- Total Cable TV – The total amount due for Cable TV Services
- Internet Services:
- The billing period for Internet services
- The itemized list of internet services including any package discounts that may apply
- Telephone Local Service:
- The telephone number that charges apply to
- The itemized list of telephone services including any package discounts that may apply
- Optional Telephone Service Features:
- The telephone number that charges apply to
- The itemized list of features for telephone services
- Long Distance:
- The telephone number that charges apply to
- The itemized list of long distance charges including number called, date, time, number of minutes, and city
- Telephone Taxes/Fees:
- Telephone service requires fees for federal and state taxes, fees and other charges. This is an itemized list of the amount billed for each.
- Total Telephone – The total amount due for Telephone services
